What is Diabetic Skin Disease?

A person having diabetes mellitus has to suffer from a skin problem during the course of his diabetes. The common skin disorders that can appear in a person with diabetes are:

 

Diabetic dermopathy

This condition is also called as shin spots as the patches appear on the shins, that is on the front side of the legs below the knees. Rarely the patches can also be found on the thighs, scalp, side of the feet or on the forearm.

This skin condition is because of the changes in the blood vessels that supply blood to the skin. There are shiny, round or oval patches on the lower front part of the legs which appear reddish brown and slightly elevated above the skin. In the beginning, the patches are slightly scaly but later become sunken.

These patches usually appear after an injury to the area.

It usually occurs in old people who have had diabetes for at least 10-20 years.

The patches of diabetic dermopathy are harmless and go off after a few years if the blood sugar levels are well controlled.

 

Diabetic bullae

In this condition, there are numerous blisters or fluid-filled eruptions on the hands and feet of the diabetic patients. It is seen in people who are suffering from diabetes for quite a long period of time or in those who have complications of diabetes like eye problems or kidney problems due to high blood sugar levels.

The blisters can range from 0.5 to 17 cms in size and usually have an irregular shape.

The bullae are of 2 types; one which is filled with clear fluid and normally heals on their own within 2-5 weeks without leaving any mark on the skin. The other type is the one which is filled with blood and it leaves a mark on the skin after healing.

The blisters or the bullae should not be broken by the patient as it can cause infection and can lead to more complications.

 

Diabetic stiff skin (digital sclerosis)

In this condition, the skin over the joints of the hands becomes thick and yellow and causes difficulty in moving the joints freely. This is because of the reaction of the increased levels of blood sugar with that of the skin proteins.

 

Eruptive xanthomatosis

In this condition, there are many firms, yellow, waxy and small lumps on the skin which are surrounded by reddish borders. They are seen mostly on the face and buttocks and rarely on the back side of legs and arms. There may be slight itching associated with the lumps.

This condition occurs because the body cannot use insulin properly to clear out the fats in the blood. The increase in levels of fats in the blood causes the formation of lumps on the skin.

Once the level of fats in the blood comes down, the lumps resolve slowly within few weeks.

 

 Necrobiosis lipoidica

This condition is caused due to changes in the collagen and fats content below the skin. The skin becomes thinner and reddish. Most of the times, the patches are found on the lower parts of the legs. These patches have a well-defined border which separates them from the normal skin. The thinned area of skin is more prone to ulceration if there is any injury to the affected part. Itching and pain may accompany this condition.
